I watched a guy make custom tiles on the diy network... how is this done and what equipment is needed

Fusing, probably.
You'll need a kiln, a saw, and a willingness to do boring things.

might be fun if you are planning to create a few special accent tiles and the buy the rest of the field tiles, otherwise probably a lot of boring repetitive work. I believe there are fuse molds for basic tiles, but you might have to create your own if you need a tile size not readily available as a mold.

So fusing is all done in the kiln if i understand correct

The hot part is done in the kiln. You will also need glass cutting equipment and a grinder.
